Putting a Door in the Doorway of a house, is just like putting a Placeable. You click on the Door you wanted and stick the model in the Doorway. The Door will automatically "SNAP" into place. (Exterior Doors are for "Exterior" of a Building, and Interior Doors are for "Interiors", as the names suggest.)
In NWN1, the door SNAPS into place and that's it. But in NWN2, the Door may/may not be hanged properly. Just like in real life, you have to hang the door onto its hinges properly, or risk having the door falls on you, it's the same in NWN2.
NWN2 Developers added a Hook Point to the left-hand bottom corner of the Doorway to help in this learning process. (Remember that you can use "Show/Hide" to "see" the Hook Points anchor.)
To hinge a Door properly, you need to hang the Doorway on the Hook Point of the Doorway. The Hook Point anchor is located on the left-hand bottom corner of the Doorway, and it is this one corner you need to click (or placed) the model of the Door to make it "stay in place".
Remember, how you would press "ESC" to get rid of the model on your cursor when you put down a placeable? If you did not "place" a door properly in the Doorway, both the model on the cursor, as well as the Door in the Doorway will "disappear" when you press "ESC"! This means you would have to put the Door into the Doorway again.
Now, if you have clicked the cursor (when putting the Door in place) right where the Hook Point of the Doorway is (and not just "anywhere" in the Doorway), the Door would remain in the Doorway when you pressed "ESC". Now you know you have done it correctly. (I know, who would have thought it is done this way?)
[Note: For Exterior Area, all house models come "preinstalled" with Doors, so there is no need to hang anything in the Doorway. Unless you wanted a Door different from the default. In that case, select the existing Door, hit DELETE to remove the default Door. The Door will disappear, leaving behind an empty Doorway. Now you can hang a new Door in place of the old one.]

(If you want, click "Show/Hide" » "Hook Points" to "see" where the Hook Point is — this is optional. Once you become used to doing this, there is no need to "see" the Hook Points, at all.)

Finally, you can name the Tag of the Door as desired.
